REGISTERING A BUSINESS

The application is checked for completeness and goes through a simultaneous evaluation by the evaluators of the Business Permits and Licensing Office (BPLO), the Zoning Office (for the locational clearance), and the Bureau of Fire Protection. 
2.    (For approved applications) Receive the Tax Bill, pay at the Cashier and receive the Official Receipt and the Business Permit and Business Plate

Note: Barangay Clearance is post-requirement for new businesses.

 


RENEWAL OF BUSINESS PERMITS

STEP 1:  Pay your BUSINESS TAX due for 2018

Quezon City Hall Taxpayers’ Lounge

STEP 2:  Go to Business One Stop Shop (BOSS) and submit the following documents for evaluation:

    2017 Business Permit
    Official receipt of business tax paid
    2017 Barangay clearance, Updated clearances and permits
    Valid  Locational Clearance, Valid Fire Safety Inspection Certificate (FSIC)

Bring original and  1 photocopy of all these documents

STEP 3:  Get claim stub which indicates “pickup date” for the renewed permit

STEP 4:  On pick up date, go to BOSS and pick up 2018 Business Permit, plate and sticker

TAKE NOTE:

 Your business permit will be revoked or automatically lapse if your permits and clearances expire and are not renewed on or before their respective expiration dates.
Fire Safety Inspection Certificates have a 365-day validity period.
Locational Clearances have a one-year validity period from date of issuance, if conditional; they have a 3-year validity period from date of issuance, if location is permissible Liquor Permits all expire on end of calendar year, December 31, 2017.
Sanitary Permits and Health Certificates all expire on end of calendar year, December 31, 2017; but may be processed for renewal within the first quarter (January to March) 2018 for CY 2018.
Tourism Registration for tourism establishments is renewed every calendar year; fees are computed as part of assessment of business taxes.


REQUIREMENTS FOR BUSINESS RETIREMENT

1.    Duly accomplished retirement application form with Location Map (3 pages/set; originally signed by the Owner/President or Vice President)
2.    Tax Bill & Official Receipts (3 years) (original)
3.    Latest Business Permit (original)
a.    If single proprietorship, original Affidavit of closure with exact date of closure
b.    If partnership, original partnership dissolution with exact effectivity date of closure signed by all partners
c.    If corporation, Original Secretary’s certificate or Board Resolution on closure or transfer of business with exact date of closure

4.    Valid ID
a.    If single proprietorship, ID of the owner
b.    If partnership, all IDs of partners are required
c.    If corporation, ID of President (original must be presented & photocopy to be submitted) and authorization letter from the owner and valid ID of the authorized representative
5.    Barangay Certificate with Exact Effectivity Date of Closure
6.    BIR Certificate of Registration (original must be presented & photocopy to be submitted)
7.    Certified Breakdown of Sales, if there are two or more line of business/branches & if AFS is consolidated (original)
8.    Proof of business tax payment/tax bill/permit issued by the other LGU where other operations are situated (original must be presented & photocopy to be submitted)
9.    Latest ITR with AFS on or before closure from the last payment of business operation for years 20___ to 20___ (original must be presented & photocopy to be submitted)
10.    VAT Returns or Percentage Tax Returns 201___ (original must be presented & photocopy to be submitted)
11.    Book of Accounts (to be presented upon evaluation/actual inspection and examination)

LOCATIONAL CLEARANCE fOR BUSINESS PERMIT PURPOSES:

For new businesses

Applicants for new businesses must apply for their Locational Clearance through the Business One Stop Shop (BOSS), ground floor, Business Permits and Licensing Office (BPLO), Center Quezon City Hall

It will be processed along with the application for New Business Registration. The applicant must bring the following documents for purposes of applying for the locational clearance for the first time for their business:

Or other arrangements for the use of the property, such as Memorandum of Agreement, formal consent of the owner for the applicant to use the property as business location

The applicant must also bring his/her identification card. If the one filing the application is not the business owner, then he/ she must bring an authorization letter from the owner.

Additional document requirement: If the location is inside a subdivision or a predominantly residential area: signed certificate of “no objection from Homeowner’s Association or Neighbor’s Consent is required.

For existing businesses renewing their registration

Check the validity of your locational clearance: For uses that are “permissible”, meaning, they conform to the provisions of the Zoning Ordinance, the validity of the locational clearance is up to three (3) years. It must be renewed every three (3) years thereafter, as long as the business continues to operate. For uses that are “conditional”, meaning the clearance was granted subject to specific conditions, the validity of the locational clearance is twelve (12) months or one (1) year, because compliance to the conditions must be regularly checked. It must be renewed every year thereafter, as long as the business continues to operate.

Before the validity date of your locational clearance expires, have it renewed by going to the Zoning Administration Unit of the City Planning and Development Office (CPDO), 10th floor, Quezon City Hall High-rise Building. You may do this as early as three (3) months before the locational clearance expiry date.

Bring the original copy of the locational clearance and the business permit for this purpose. For easier processing, attach the locational plan/ subdivision plan/ sketch showing exact position and area occupied by the business being applied for.

 STEPS FOR SECURING AN OCCUPATIONAL PERMIT WITH THE BUSINESS PERMITS AND LICENSING OFFICE

Legal Basis:
“Article 22 of the Quezon City Revenue Code as amended:
Section 83. It shall be unlawful for any person to exercise his occupation or calling within the jurisdiction of the city, without acquiring the individual Mayor's Permit.
Section 84. The Mayor's Permit fee on occupation or calling shall be paid to the city Treasurer before any occupation or calling can be lawfully pursued in the city."

Before applying for Occupational Permit at BPLO, you must first secure a Health Card from City Health Department. Once you have secured your Health Card, you can now apply for Occupational Permit.

If you’re applying for Occupational Permit for the first time, you must first secure a Health Card from City Health Department. Once you have secured your Health Card, you can now apply for Occupational Permit.

FOR NEW and RENEWAL APPLICANTS

1.    Duly accomplish Occupational Permit Information Sheet at Business Permits and Licensing Department. Attach and submit the following documents with your information sheet:

  • National Bureau of Investigation or Police Clearance (latest)
  • Cedula (latest)
  • Health Card and Health Official Receipt
  • Yellow Cards (for entertainers and therapist)
  • Waiver for Professional Regulatory Commission (PRC) card holder 
  • Parent’s Consent (if minor)
  • Alien Employment Permit (if foreigner)
  • Department of Public Order and Safety Clearance (for security guards)

2.    Get your Order of Payment and pay at the cashier.
3.    Claim your Occupational Permit.

Guidelines and Fees on Securing a Health Certificate

Legal basis:

“Article 30, Section 132 of the Quezon City Revenue Code, as Amended states:
"all persons employed in business establishments engaged in food, entertainment and personal services are required to undergo medical and physical examination given by government hospitals, medical clinics and those private hospitals and medical clinics that may be accredited for the purpose..."

PROCESS OF APPLICATION

Requirements for first time applicants:

1.    Get your Order of Payment and pay Php 194.00 for your Health Card at the City Treasurer's cashier.
2.    Submit for laboratory examination, your stool and sputum specimen and get the results. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories (please see listing)
3.    Attend an HIV/AIDS seminar
Individual applicants can attend the HIV/AIDS seminar on the spot; however, it is a First Come First Serve basis.

Companies can apply for the Health Card and Occupational Permit Card for all their employees altogether.

Companies can request to:

1.    Have their employees' stool and sputum examination done at any accredited laboratories
2.    Have their HIV/AIDS seminar done for their employees at their company premises and the City Health Department will send the lecturer/s at the agreed upon schedule.

Companies must:
a.    Submit their employees' photos for their Health Card. If the photo-taking is not done at the Health Department, the ID provided will be a "white" Health Card, and not a self-laminated card. This card is good only for one year, while the self-laminated card has provisions for 3 years use.
b.    Get their employees’ Health Card.

Requirements for renewal of Health Card:

1.    Get your Order of Payment and pay Php 144.00 for your Health Card at the City Treasurer's cashier.
2.    Submit for laboratory examination, your stool and sputum specimen and get the results. The stool and sputum specimen can be submitted to the City Health Department or to any accredited laboratories
3.    Bring your old Health Card for stickering at the City Health Department (your Health Card is good for 3 years)

 Additional requirements for:
a.    Applicants below 18 years old:

  • Parental consent
  • Birth certificate/baptismal certificate
  • Valid ID of the parent/guardian giving consent

b.    Massagists/ Therapist/Massage Clinic Attendants/Reflexologists, etc:

  • Massagists course
  • Original Birth Certificate issued by NSO
  • Marriage Certificate (if married)
  • 2 pc. 2x2 w/ white background
  • 2 pcs. 1x1 w/ collar & sleeves
  • Lab. Result : VD Clearance
  • Official Receipt (P 309.00)
  • Massagists’ Training Certificate
  • Date of training: Every 3rd wk of the month Tuesday to Friday; 1-5PM

c.    GRO/FRM/DANCER/ENTERTAINER, etc.:

  • Original Birth Certificate from NSO/ Local Civil Registry
  • Marriage Certificate (if married)
  • 1x1 ID picture
  • Lab. Result : VD Clearance
  • Official Receipt (P 259.00)

 For New Applicants of Liquor License:
1. File an application in prescribed form of Quezon City's Liquor Licensing & Regulatory Board (LLRB).
2. Present the original copy of your 2018 business permit and official receipt of payment of Permit and License Fees. A photocopy of these is also required for filing purposes of LLRB.
3. Business establishment will be subject to physical and on-site inspection by authorized LLRB inspector. A corresponding inspection report will then be submitted to the LLRB Office for processing and approval of application.
4. Upon approval of the application by the LLRB Board, an order for payment of License Fee will be given to the applicant to be presented at the Taxes & Fees Division of the Treasurer's Office for final assessment and payment of fees.
5. Upon payment, an official receipt will be given to the applicant by the Treasurer's Office. Applicant will then submit to LLRB the official receipt together with a photocopy of same for proper recording and final processing of application.
6. Completed documents will be reviewed by the assigned processor before final action of the LLRB Executive Director for submission to the Chairman, LLRB for final approval.
7. After approval, the Liquor License will be properly recorded and compiled and released to the applicant.
REQUIREMENTS FOR NEW APPLICANTS OF LIQUOR REGULATORY PERMIT
1.    BPLO Permit (photocopy)
2.    BPLO Official Receipt & Assessment (photocopy)
3.    Copy of SEC Registration, Articles of Incorporation & By-Laws, if corporation
4.    Photo of front & inside of establishment (for server only)
5.    Barangay Clearance for Liquor
6.    Contract of Lease

Please note that for new business applicants, Original BPLO Permit, Official Receipt & Assessment form must be presented.

      For Renewal of Liquor License:
1. Submit application form for renewal at the LLRB Clearance Section for processing.
The application, together with supporting documents (2018 business permit and previous year's Liquor License), will be reviewed by the Processor before action is taken by the LLRB Executive Director.
2. The corresponding Order of Payment for liquor license will be issued to the applicant for payment at Taxes & Fees Division of the City Treasurer's Office.
3. Official Receipt of payment together with a photocopy of this should be submitted to LLRB for recording and final processing.
4. Prepared and processed Liquor License shall be submitted to the LLRB Executive Director for proper action and recommendation for approval by the LLRB Chairman.
5. After approval, Liquor License is ready for release to Licensee.
